Physician on Call:
For your convenience a physician is on call twenty-four hours a day, seven days a week. After hours, one of our physicians is available to give advice for emergency calls. However, we do not prescribe medication or give refills over the phone.
During regular office hours, our clinical case managers are available to handle any questions you may have.
If you have a life threatening medical emergency, call 911. If after our regular working hours your child needs to see a physician, please take your child to the Scottish Rite Emergency or an urgent care center. For directions to Scottish Rite Emergency, click here.
Prescriptions can only be refilled during normal office hours. The on call physician will not prescribe or refill any medication after office hours. When calling in for a refill, the required time needed is 24 – 48 hours to complete your request. Our office will make every effort to handle your request in the shortest time possible. When you are calling to renew a prescription, please leave the name and the phone number of your pharmacy along with the name of the medication, dosage and quantity. No prescriptions for any scheduled medication such as narcotics or controlled drugs such as Ritalin or Adderall (ADHD meds) will be renewed over the phone.
Appointments & Cancellations:
We make every effort to honor all our commitments and request that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. It is extremely important that all patients honor their reserved appointments. Failure to do so deprives other children from receiving needed care in a timely fashion. Our office policy stipulates that failure to give sufficient notice of inability to keep a scheduled appointment (48 hours advance notification during office hours) will be charged a minimum of $25 up to $150 depending on appointment type. This charge is in accordance with our office policy for all patients and is to be paid prior to the scheduling of a new appointment. In the event you arrive 15 or more minutes late for an appointment, we reserve the right to reschedule your appointment for another time.
It is our policy that the patient must be present at every appointment.
Insurance & Payment Policy:
If you are insured by one of the plans in which we are providers, we will make every effort to comply with your insurance company’s rules. You must have your plan ID card at the time of each visit and must be prepared to pay your co-pay, deductible or services that are not covered by your insurance. The parent or guardian who accompanies the child to the visit is responsible for payment. For your convenience we accept cash, check, MasterCard, Visa or Discover. You can use the secure link located on the homepage of this web address to pay on-line. If you are on a plan for which we are not providers, our business office will be happy to provide you with the necessary paperwork for you to file with your insurance company after services are rendered. Our business office is available to answer any questions you may have regarding your account. However, due to the many different plans, each with its own regulations and different allowable benefits, we cannot assume any financial responsibility for estimation of benefits your insurance may give us.
Medical Forms Completion:
There is a $40.00 yearly form completion fee for the completion of medical forms or a onetime fee, per form of $10. When requesting a medical form for completion for your child(ren)’s school or daycare, please be advised that the required time needed to complete your request is 7-10 business days. We will make every effort to complete your request in the shortest time possible.
When requesting your child(ren)’s medical records for yourself, or to be transferred to another physicians office, please be advised that the required time needed to complete your request is 7-10 business days. If requesting the last office note only to be faxed or mailed, the office note will be sent at no charge. If you are requesting the entire medical record, there will be a fee per child in accordance with Georgia Statute 31-33-3. We will make every effort to complete your request in the shortest time possible. We use an outside service, Healthort, to copy and send out medical records. Referral requirements vary by insurance carrier. It is not possible for us to keep up with the hundreds of policies and plans our patients are covered under. It’s the responsibility of the patient to make sure they’ve obtained the referral required by their plan, and to make sure the referral is renewed or extended when necessary.
